I got a similar cable, but NEMA5-15P / IEC - C19 a few years ago from 
Quail Electronics (http://www.quail.com/).  The cable i got is on page 
77 of their catalog, and the one you pictured above is on page 79.  I 
emailed and asked for a price on the cable, and they emailed back asking 
what quantity i wanted.  I replied and said that i was a hobbist, and 
just needed one, and they replied saying that they would send one for 
free.  A few days later it showed up.  I certainly would not expect that 
again, but it's probably worth checking their price to see if you get as 
lucky as i did.
