Matrox makes one called the QID:

Be warned, they cost $600 US.

Dual dual-head video cards (one AGP, one PCI) might be more cost effective.

You can also try to find one using the nVidia Quadro4 NVS440 chip, but I
don't think those exist in anything but a PCI-E card.
