The incremental cost is barely above zero, yes, but to what extent do
you amortize all the setup and (self-)education time?

Not all that much of a myth, for "most people". When your expertise is
car body repair, or farming carrots, or addressing inner-city gang
violence? Then the work of running a mailserver _would_ be onerous and
sizeable, because it would have to include all the work of learning to
sysadmin a computer _at all_ and suchlike. To some extent even if your
expertise in computers, when it is, say, writing RPG for corporate
payrolls, or simulating airflow in the presence of ice buildup on
airplanes.
Of course, how relevant that is to people already on this list, that is
a separate question.
