Since you mention ISDN: Some of the Osmocom people are working - 
very casually - on a stack to transport E1 over IP, and building a 
ISDN telephony network on that. I haven't seen analog modems mentioned 
in particular, but when ISDN works, transporting analog telephony 
should be not much of a problem. I'd assume you'll still need an 
IP network with relatively low latency and jitter underneath, but they 
seem to have a system that connects people in several different cities  
all over Germany.

In addition to software and a decent-sized PBX or other device that 
does E1, the setup also requires specialized hardware, particularly an 
Osmocom icE1usb: https://sysmocom.de/products/lab/icE1usb/

(Open source hardware, but available pre-built at 
http://shop.sysmocom.de/products/icE1usb - not exactly cheap.)

I have no idea if the setup can somehow be tweaked towards T1, which 
would probably be more useful for gear available in the US.
