I've had good luck with Znyx cards... although I used them in Suns, not
PCs. They actually worked better in some lab settings than the Sun qfe
cards. 

Also had good luck with Intel dual port cards under linux, bsd, and
Windows. Usually get those for $50 or less these days.

You can find some quad tulip Osicom cards on ebay very cheaply ($100ish?).
