[rescue] 4/110 workingness

I dusted off a 4/110 motherboard tonight, and discovered two valuable
pieces of info.

1.  I was getting an 'unexpected interrupt 0xf ...' error on diag test.
That was the
result of having a few 8 chip simms in one of the banks.  Replace the 8
chip simms
with 9 chip and everything works fine.

2. For two banks of 1M simms (16M), fill banks 1 and 3 on the FAQ
diagram,
not banks 1 and 2.
