Thanks for collecting these and keeping them online! 

I won't rule out a hardware problem - my card has basically been sitting in a drawer for 20 years or so after it was discarded as unknown hardware... My version also doesn't have the jumpers that are being mentioned in the FAQ and the docs.

I spent a couple of hours these days trying to turn up other versions, or even the same from somewhere else, but it doesn't seem there's anything archived anywhere (or at least not within the reach of the usual search engines).
